Changes since last release:

Fix for odd number of cylinders with dual+missing triggers
Minimum pulse width setting. Separate from deadtime. Allows the user to shut off injectors below the pulse-width where the injector will never open.
Staged injection checks if secondary pulse-width is below min pulse width, and doesn't stage if that is the case.
Fix bug with staged injection that caused it not to work in table-based mode when <= 10% staged.
Fix VVT for several engine types.
Fix default lambda values (previously off by 10x)
Make dome control "Simple" mode work.
Huge code clean-up (start of MISRA compliance)
Removed dash selector
Fix a bug where Comms chip didn't try to communicate with the engine chip for
2 seconds after power-up. (Potentially fixes startup issues immediately after power-on when laptop was disconnected)
Fix for CAN real-time broadcasting issue that would stop broadcasting after around 9 seconds.
